PMR Centre ("we", "us", "our") is a Physical Medicine and Rehabilitation clinic in Shillong, Meghalaya, India. We take the privacy of our patients and website visitors seriously. This policy explains what personal information we collect, why we collect it, and the choices you have.

Information we collect
Clinical and patient information. When you register as a patient or attend an appointment, we collect your name, date of birth, gender, contact details, address, and the clinical information needed to provide your care — including your medical history, examination findings, investigation results, prescriptions and treatment records. You are assigned an MRN (patient number) that identifies your record.
Patient Portal. If you use our Patient Portal you sign in with your MRN and date of birth, and we send a one-time verification code to the mobile number registered with us by WhatsApp and SMS. We keep a record of portal sign-ins and the pages accessed, as an audit trail for the security of your own records.
Enquiries and forms. If you contact us through a form on this website, by email, or through the chat window, we collect the details you provide so we can respond.
Payments. Online payments are processed by our payment gateway. We receive confirmation of the payment, the amount and a transaction reference. We do not receive or store your card, UPI or bank credentials — those are handled entirely by the payment provider.
Technical information. Our web server records standard information such as IP address, browser type and pages requested, which we use to keep the site secure and working.
How we use your information
- To provide, plan and coordinate your clinical care and rehabilitation.
- To manage appointments, and to contact you about them.
- To issue and settle bills, and to process refunds where they apply.
- To operate the Patient Portal and telemedicine consultations.
- To answer your enquiries.
- To meet our legal, regulatory and professional record-keeping obligations.
We do not sell your personal information, and we do not use your clinical information for advertising.
Who we share it with
We share personal information only where it is necessary:
- with clinicians and staff involved in your care;
- with our payment gateway, to take payments and process refunds;
- with messaging providers, to deliver appointment and verification messages by WhatsApp and SMS;
- with other healthcare providers where you are referred, with your knowledge;
- where we are required to do so by law, a court, or a regulator.
These providers act on our instructions and are not permitted to use your information for their own purposes.
How we protect your information
This website and the Patient Portal are served over encrypted (HTTPS) connections. Access to patient records is restricted to authorised staff, portal access requires a one-time verification code in addition to your MRN and date of birth, and portal activity is logged. No system can be guaranteed completely secure, but we take reasonable technical and organisational measures to protect your information.
How long we keep it
Clinical records are retained for a minimum of five years, and longer where applicable medical record-keeping, legal or regulatory requirements in India call for it — for example where a record relates to ongoing treatment or a matter under enquiry. Enquiry correspondence and website logs are kept only as long as needed for the purpose for which they were collected. When information is no longer required, it is securely deleted or anonymised.
Your rights
Subject to applicable law, including the Digital Personal Data Protection Act, 2023 and the Information Technology Act, 2000, you may:
- ask for a copy of the personal information we hold about you;
- ask us to correct information that is inaccurate or incomplete;
- ask us to erase information, where we are not required to keep it;
- withdraw consent for non-essential processing, such as marketing messages;
- raise a grievance with us about how your information has been handled.
We may need to verify your identity before acting on a request, to make sure we do not disclose records to the wrong person. Requests and complaints about your personal information should be addressed to our Grievance Officer, named below.
Cookies, chat and analytics
This website uses a small number of cookies that are necessary for it to work, such as keeping a form session active. The chat window on our site is provided by our own chat service and may set a cookie so a conversation can continue between visits. If website analytics is enabled, it is used only to understand how the site is used in aggregate — not to identify you personally. You can block or delete cookies in your browser settings; essential features may not work if you do.
Children
We treat information about children with the same care as any other patient information. Where a patient is a minor, we deal with a parent or legal guardian.

Our website links to other services, including our Patient Portal and our social media pages. Those services have their own privacy policies, and we are not responsible for their content or practices.

We may update this policy from time to time. The version published on this page is the current one, and the review date is shown above.
